The Advance ProjectTM recognises that GPs and nurses working in general practice have the potential to have an important role in advance care planning and palliative care. The Advance ProjectTM has identified tools that can be used to initiate advance care planning discussions and screen for supportive care needs that can be used in the general practice setting.
