We've upgraded the tools in the Advance Project Toolkit

The Advance Project Toolkit consists of a number of resources for primary care clinicians, and also patient and carer assessment tools. They are designed to help you to work as a team, to initiate conversations about advance care planning, and to assess patients’ and their carers’ palliative and supportive care needs.

Online training modules are also provided, which explain how to use these resources in everyday clinical practice. The training will enable you to undertake screening and assessment of patients to:

  • Sensitively promote awareness of advance care planning with older and/or chronically ill adults
  • Identify those patients who may be at risk of deteriorating and dying
  • Assess these patients’ symptoms, address important questions and concerns, and identify their carers’ concerns
  • Identify patients who might most benefit from referral to a specialist palliative care service.

How was it developed?

The Advance Project Toolkit was informed by literature reviews of the best available evidence about tools to support palliative care and advance care planning in general practice, as well as extensive input from our expert advisory group, GPs, general practices nurse, practice managers and consumer representatives.

It has been officially recognised as an Accepted Clinical Resource by the Royal Australian College of General Practitioners (RACGP) and endorsed by the Australian Primary Health Care Nurses Association (APNA) according to approved quality standards criteria.
